Official School Report Cards and Transcripts

K-8th Grade Classes
Serving Institute requires official report cards from the last school that your student has attended, regardless of the grade he/she will be entering. An official signature or embossed seal is not required for K-8 report cards/transcripts if high school credit has not been awarded. If high school credit has been awarded, a signature or seal is required on the report card. All official report cards and/or transcripts (scan copy) needs to be attached via the student application form on the website.

9-12th Grade Classes
Serving Institute requires official transcripts from the last school that your student has attended, regardless of the grade he/she will be entering. Transcripts for grades 9-12 are only considered official if they are signed by a representative from the school administration and/or embossed with the official school seal. In addition, the transcript must indicate the number of credits awarded for each course, and all official report cards and/or transcripts (scan copy) needs to be attached via the student application form on the website.

Dual Enrollment Classes
Official transcripts are not necessary for students only enrolling in Serving Institute Dual Enrollment courses; however, unofficial transcripts are required.
Official Homeschool Report Cards and Transcripts
Students who have been homeschooled must provide official report cards and/or transcripts signed, with a physical or digital signature, by the parent/guardian, or provide homeschool records, reflecting all courses completed, credits earned (or hours spent) and the letter grade earned. Courses should be categorized according to the intended grade level. For guidance, view a sample homeschool transcript and sample homeschool report card. Additionally, all official report cards and/or transcripts (scan copy) needs to be attached via the student application form on the website.
Unofficial Report Cards & Transcripts
For initial evaluation of transfer credits, unofficial report cards and transcripts can be scanned and submitted in the student application form on the website. Please note that Serving Institute may choose to enroll a student using unofficial student records. However, they do so with the understanding that official student records will be provided to Serving Institute before the end of the academic term in which the student has been enrolled.
Further, Serving Institute will not award credit for completed coursework or provide official student records until the student’s official records have been received from the previous school attended.
Birth Certificates, Parent Information
Form and Ministry Reference
- Serving Institute requires student birth certificates, regardless of the grade he/she will be entering.
- Serving Institute requires a filled-out parent/guardian information form, regardless of the grade he/she will be entering. (Download form here)
- Serving Institute requires a confidential ministry reference from the pastor of your congregation, regardless of the grade he/she will be entering.
